    We provide culturally-affirming, trauma-informed support to individuals with mental health disorders to access resources, achieve independence & recovery.  Case Management How We Help Our culturally-affirming, trauma-informed case management programs support individuals with mental health disorders in accessing resources and achieving independence and recovery. Levels of Support Recovery and independence are different for every individual. Rooted in the belief that mental health should not stand in the way of reaching your goals, our case managers will work with you to create meaningful, achievable goals and help you reach them. At every level of support, common services include traditional case management, connections to resources, systems navigation, medication management, and therapy. Assertive Community Treatment (ACT) ACT is an evidence-based model for high-intensity, wrap-around case management. Community Support Team Community Support Team provides high-intensity, case management. Community Connections Community Connections focuses on building supports and independence. "The services I receive are excellent and help me reach a sense of independence. They have impacted me in several ways, mainly with my mental stability and a sense of how incredible life can be. I'm not afraid to ask questions, and I always get the help I need.” - Community Support Team Participant Eligibility & Enrollment Our case management programs serve Medicaid-eligible individuals in need of mental health services. Please note, while we may connect participants to housing resources, this is not a housing program. These programs serve individuals with mental health needs beyond housing alone.     Jourdan Sorrell is the Senior Manager of External Affairs at Comcast Corporation. < Back Jourdan Sorrell Jourdan Sorrell is the Senior Manager of External Affairs at Comcast Corporation. In this role, he leads Comcast's foundation giving and a broadband internet adoption program with school districts and other organizations sponsoring families internet at home for the company's Greater Chicago Region. His work focuses on initiatives that ensure everyone has connectivity and builds their digital skills to participate and excel in an increasingly digital economy. Before Comcast, Jourdan served as Director of Public Policy for Cook County Commissioner Stanley Moore, 4th District. In this role, he provided policy guidance on the 2020 Census, contract compliance, criminal justice reform, and real estate development. He also worked in the Cook County Clerk's Office, LINK Unlimited Scholars, and the Chicago Community Trust. He is active in civic organizations such as the 100 Black Men of Chicago and the United Way of Metro Chicago's Corporate Social Responsibility Executive Committee. He serves in nonprofit leadership roles as Board President of the South East Chicago Commission and founding President of the Rotary Club for Social Impact and Equity.     < Back Lillian Bui Senior Director of Development and External Affairs Lillian Bui (she/her) proudly serves as Development Director, setting fundraising and external affairs strategy for a team responsible for raising more than $2 million in private funds annually. Lillian brings 15 years of experience in non-profits of all sizes and is a Certified Fund Raising Executive (CFRE), an international credential earned by demonstrating mastery through education, professional practice, and performance. Before Association House, Lillian served as the first full-time development staff person at the AZA-accredited Pueblo Zoo. There, she completed an eight-year capital campaign and grew the program from a one-person shop to a four-person team inclusive of fundraising, marketing, events, and membership. Her collaborative and strategic approach drives organizational efficiencies and growth. She believes in the power of vision and transparent communication and is dedicated to helping others reach their goals. An ambitious and avid learner, Lillian holds a Masters of Arts in Public Policy from the University of Chicago Harris School of Public Policy and a Bachelor of Arts in History from Lake Forest College. Humboldt Park holds a special place in Lillian’s heart, as she began her professional career as an AmeriCorps volunteer with City Year Chicago at the former Lafayette Elementary at the intersection Augusta and Washtenaw. As a fundraiser, she is passionate about growing programs that address the economic, educational, and racial disparities that keep people from reaching their fullest potential.     < Back Lincoln Stannard Senior Director of Community Development Lincoln is a passionate community development practitioner and housing advocate. Over 7 years at LUCHA, he has overseen accomplishments including the completion of Tierra Linda and groundbreaking of Encuentro Square. As Director of Community Development, he has grown a project pipeline with the greatest number of units under development in LUCHA’s history. He brings a mission-centered approach, strong analytical skills and deep experience with a variety of project types and financing models. Lincoln began his career at the Chicago Community Loan Fund, where he managed construction lending for the City of Chicago’s Neighborhood Stabilization Program. He holds a Grow America Economic Development Finance Professional certification. Born and raised on Chicago’s West Side, Lincoln has called Logan Square home for 14 years. He has been active in local housing justice and tenant organizing projects, including time spent on the boards of the Here to Stay Land Trust and Somos Logan Square. An avid biker, his favorite destination in the city right now is the Montrose moonrise observation point on the lakefront.
