  • Healthy Lifestyles | Association House

    Health Education How We Help Healthy Lifestyles is our health education program that aims to increase awareness, knowledge, and skills for healthy practices to help decrease risk factors for nutrition-related health problems and chronic diseases through a series of adult and child-friendly workshops. Depending on the series, each workshop ranges from 30 to 60 minutes. Cancer This workshop provides an overview of several different types of cancer, including lung, skin, breast, and prostate cancers. We cover risk factors, preventative measures, treatment, and diagnosis. We discuss how food and healthy eating can affect our health and influence our health outcomes. Cooking demonstrations include healthy recipes, tips on making smarter choices for takeout, and understanding food labels. Resources from National Cancer Institute, CDC, Mayo Clinic, Harvard Health, Cancer Center. Diabetes This workshop provides basic information, symptoms, and everyday diabetes care for newly diagnosed people. Topics include Type I, Type II, and Gestational Diabetes. Workshops cover risk factors, controlling blood sugar levels, and possible treatments. Our cooking demonstrations consist of diabetic-friendly and low-sugar recipes. ​ Resources from American Diabetes Association, CLOCC, Mayo Clinic, CDC. Asthma and Chronic Obstructive Pulmonary Disease These workshops provide background knowledge and education on asthma, allergies, and chronic obstructive pulmonary disease. Participants will be able to recognize symptoms, risk factors, treatments and management skills. We also discuss the range of food and seasonal allergies. Cooking demonstrations incorporate finding and swapping out certain ingredients to manage any food allergies. ​ Resources from Harvard Health, CDC, American College of Asthma, Allergy, and Immunology, American Lung Association. Cardiovascular Disease These workshops include learning more about the heart and how to take both preventative and proactive measures for heart disease, cholesterol, high blood pressure, and strokes. We cover heart-healthy foods that can help with specific heart issues and concerns. Cooking demonstrations include heart-friendly recipes. ​ Resources from American Heart Association, Harvard Health, CDC. Mental Health First Aid Mental Health First Aid is a nationally recognized training that teaches participants how to identify, understand, and respond to signs of mental illness in their family, friends, community, and workplaces. We offer Adult and Youth Mental Health First Aid Trainings in English and Spanish, in-person or virtually. The in-person training requires an 8-hour time commitment. The virtual training is six hours with pre-work required. Learn More “Many of our participants do not have access to mental health resources where they live. Some people do not even know where or how to seek help. Other individuals may not be willing to talk about their mental health issues due to stigma. The skills and tools learned from the MHFA trainings provide individuals with the skills, support, and resources needed to aid a person that is going through a potential mental health crisis.     Dulcinea How We Help We offer a structured living setting for adults with severe and persistent mental illness or co-occurring mental health and substance use disorders. The program’s overall goal is for participants build their skills to function independently and confidently in the community. What to Expect Independent living, social, adaptive and coping skill building activities that include groups and individual sessions Medication Monitoring and Training Case Management and Community Support Crisis Prevention and Intervention Services Access to psychiatric and medical care Individualized treatment plans to support goal achievement Opportunities to develop natural supports Eligibility & Enrollment This program serves Medicaid-eligible individuals with severe mental health or substance use disorder. Individuals must be enrolled in a referring Association House program. If you are interested in learning more, please contact us at 773-772-8009 ext. 1020. ​ Adults 18+ years Adults with a mental health diagnosis or co-occurring mental health and substance use disorder. Adults with functional living needs who require assistance with activities of daily living and support to transition into the community. Medicaid and source of income (SSI/SSDI) Do not require 24 hour medical attention Ability to self-administer own medications Ability to ascend and descend stairs without assistance (there are stairs to enter the building and to access the floors with bedrooms). History of psychiatric hospitalizations “When I first came here, I was going through hard changes because I had nowhere to live. I was so happy that they (AHC) gave me a hand, It felt like they understood me. When I was in the hospital I lost my apartment, and they gave me a place to stay. I made friends with lots of people here, they helped me a lot. ” - Association House Participant 88% of participants improved their Independent Living Skills in order to obtain their main goal of moving into the community independently. This includes facilitating career assessments, developing career plans, matching applicants to our various training programs, connecting program graduates with job opportunities, providing job retention services, and maintaining current knowledge of industry and hiring trends. Requirements High School Diploma or GED required. Associates degree from an accredited college or university in a field related to program services and outcomes preferred. Minimum of 3 years of professional adult training experience (trainer, teacher, instructor, or educator). Demonstrated ability to effectively interact with a diverse participant population. Experience in recruiting, assessing, enrolling, career coaching and placing participants strongly preferred. Background in retail branch banking preferred. Strong customer service orientation and results orientation. Demonstrated sales skills strongly preferred. Bilingual English/Spanish preferred.     < Back Margarita Rosas Director of Finance Margarita Rosas (she/her) serves as Director of Finance. Reporting directly to the President, Margarita collaborates with the other members of the leadership team, as well as the Board of Directors, to support the financial goals of the organization managing a $17M budget. Margarita brings 20 years of experience in process improvement, systems design, project management, strategic financial planning, monitoring and evaluation, public finance & policy planning across various organizations at the local, national, and international level. Before joining Association House, Rosas served as the Director of Finance with New York Sun Works, driving financial functions and performance. In her five years, Rosas was instrumental in expanding the organization from 90 to over 400 school partners, impacting 120,000+ students annually. Born and raised in Colombia, Margarita has been an international consultant for the International Monetary Fund, deputy director for social investments with the Colombian national government, and Founder of New Life for Everyone, a foundation that supports educational opportunities for over 400 children in marginalized communities in Bogotá. Margarita holds a Master’s of Public Administration from Cornell University, a Master’s in Regional Planning with an emphasis in Local Economic Development, and a dual BA/BS in Business Administration and Economics, both from Universidad de Los Andes. As a Latina, an immigrant, and a mother of a child with disabilities, Margarita feels strongly connected to Association House’s mission for inclusion and creating opportunities for marginalized communities. ​ ​

I have been an educator for many, many years but have been in leadership for 7 years. What inspires you as an administrator? I continue to be inspired by the opportunity to create change in the lives of the youth I serve. I also continue to be inspired by my passion and desire to take on challenges when they arise. Thirdly, I continue to be inspired by success stories of the youth I have served and those I am serving. What is one fun fact about yourself?   • Damaris Acevedo | Association House

    < Back Damaris Acevedo Director of Child Welfare Damaris currently oversees the Child Welfare Division, which provides quality services for more than 200 active family cases, including foster care, intact family services, and adoptions. She is a Licensed Social Worker with an Illinois Department of Children and Family Services license and is 402 & CCA certified. Since 2005, Damaris has worked in the Child Welfare Division and began her tenure as a foster care case manager. Damaris attended the University of Illinois at Chicago’s Jane Addams School of Social Work, where she received her Bachelors of Social Work; and received her MSW from Dominican University. Damaris also obtained her LCSW in 2017. Damaris is passionate about helping families, and her commitment is to ensure we treat every participant with dignity and respect regardless of what brought them to our program. She says her work is difficult but very rewarding. ​ ​

  • Tanya A. Wade | Association House

    < Back Tanya A. Wade Clinical Director Since 2014, Tanya has served as our Clinical Director where she monitors program performance, provides clinical supervision and direction, and ensures participants receive high-quality clinical services and care. Tanya is a Licensed Clinical Social Worker (LCSW) and originally joined Association House as a Continuity of Care Coordinator in May 2013. Tanya graduated from Hofstra University with a Bachelor of Arts degree in English. In 1998, Tanya moved to Chicago and received a Master of Social Work degree from Loyola University Chicago. Tanya then honed her clinical skills as a trainer, diagnostician, and therapist within Child Welfare and Community Mental Health. Tanya works at Association House because she has a personal commitment to clear the path for participants and staff alike to realize their full potential. She believes that Association House leads by example; opens doors, encourages ideas, nurtures healing, and empowers change. ​ ​
