About the Role
Responsible for assisting individuals residing in nursing facility care to prepare for and implement steps to transition to placement in community. Under the guidance of the Quality Administrator, supervises transition team staff and provides day-to-day task assignment and monitoring to ensure quality of care, documentation, and productivity. Implements comprehensive service plan recommendations; provides and oversees SOAR services and transition/linkage services. Serves as a liaison with other systems of care, including nursing facility staff, care managers, medical providers, SLFs, sub-recipient teams, and collaborating treatment providers. Documents all services using program standards.
Requirements
Bachelor’s degree from an accredited college or university in psychology, social work, or a related field (or) Bachelor’s degree plus 2 years of supervised experience in a mental health setting (or) Master’s degree from an accredited college or university in a related field (preferred).
Experience working with persons with psychiatric disorders, substance abuse, development disorders required.
Experience working with diverse populations required.
At least 2 years working with the consent decrees preferred.
Personal vehicle, Illinois current driver license and insurance, good driving record acceptable to agency insurance carrier.
Always carries cellular phone to respond to participant/program needs.
Flexibility to work on weekends, evenings, holidays during emergencies.
