About the Role
Provides oversight, reporting, charting, coordination, leadership, training to staff, and evaluation of the Comprehensive Class Member Transition Program (CCMTP). Oversees various program and documentation needs for CCMTP, life skills, in-house service provision and CCMTP programming. Provides guidance to RN and OT staff in reviewing, and providing feedback on documentation, updates to applicable policy changes, life skill supply requests, coordination of care with external providers and maintaining working relationships with nursing facility staff. Completes care management assessments and service planning for processes using Caspio Web Application. Represents agency and program effectively in the community, and assists in building collaborative relationships with other providers, teams, facilities, and entities
Requirements
Master’s Degree from an accredited college or university in psychology, counseling, social work, human services, or related field. Clinical licensure preferred (LCSW, LCPC, LMTF).
One year of direct experience in community behavioral health setting (required).
Experience working with diverse populations (required).
Must have daily use of a car, current insurance, valid driver’s license, and a driving record acceptable to Agency insurance carrier.
Availability to work both on-site and in the field on a full-time basis.
Carries cellular phone at all times to respond to participant/program needs.
