Clinical Supervisor – CCMTP

About the Role

Provides oversight, coordination, leadership, training, and evaluation of Comprehensive Class Member Transition Program (CCMTP) Care Management, RN and OT Staff. Oversees all administrative and clinical aspects of CCTMTP assessment and service planning processes, including tracking assessments and completion dates, training staff to complete assessments and service plans using Caspio Web Application, reviewing, and providing feedback on clinical documentation, and developing and maintaining working relationships with nursing facility staff. Monitors delivery of care management, RN, and OT services to Class Members. Provides direct assessment, service planning, care management services, and direct case management services to Class Members as needed. Documents all services using program protocols. Represents agency and program effectively in the community, and assists in building collaborative relationships with other providers, teams, facilities, and entities.

Requirements

  • Masters degree from an accredited college or university in psychology, counseling, social work, human services, or related field.  

  • Clinical license preferred (LCSW, LCPC, LMFT).  

  • One year of direct experience in community behavioral health setting required.  

  • Experience working with diverse populations required.  

  • Must have daily use of car, current insurance, valid driver’s license, and a driving record acceptable to Agency insurance carrier.  

  • Flexibility to work on weekends, evenings, holidays during emergencies when assigned to the rotating emergency cellular.
